Player Registration
I acknowledge: Fees payable by cheque are made out to Hay River Minor Hockey Association. Fees can be paid by credit card online at http://hayrivermha.rampregistrations.com/. Fees of $50 are due prior to the player going on the ice with the remainder being due no later than November 15th. If fees are outstanding from the previous season, the player cannot step onto the ice until they are paid in full.
I acknowledge that I have read and understand HRMHA Refund Policy is as follows:
HRMHA must take the insurance portion ($50) from the registration fees as soon as the child steps on the ice. The rest is pro-rated by month. If child goes on the ice at all and decides to quit, the parent forfeits the $50 insurance fees and the refund amount would then be calculated accordingly to the appropriate monthly rate. Written notice to the divisional director must be submitted in order to qualify for a refund. If you or your child elects to quit after December 31, there is no refund for the remaining 3 months unless for major medical reasons and subject to the discretion of the HRMHA Executive.
I will also do my best to volunteer to assist in making this hockey season a great experience for my player and all players involved in HRMHA.
I acknowledge that I have read and signed the HRMHA Fair Play Code, and my child has as well. If my player is a Bantam or Midget we have read and signed the Code of Conduct.
I acknowledge that I have read and signed the HRMHA COVID-19 Waiver and will abide by the rules put in place to maintain a safe and healthy environment for all players and parents.
I acknowledge that I have filled out the required Medical Form for my child.
I acknowledge and authorize HRMHA and/or any of its members or sponsors to submit photo’s of my child for promotional purposes directly related to hockey and/or HRMHA and may be displayed on any media such as the HRMHA website, in newspapers, on posters, etc.
